Currently all information forms are to be completed at my office prior to your first appointment. Please arrive approximatey 15 minutes early. If you have insurance, please call your provider before your appointment and ask if you need a precertification number, have a deductible to meet, or a co-pay. EAP coverage only requires a precertification number. Please bring a copy of your insurance card for your file.
